Recommendation Letter Qs

If I have agreed to write a letter of recommendation for you, then please send me your responses to the questions below. I suggest you either cut/paste these into an email or send me a pdf of your responses. If you have related personal statements and a CV please feel free to include that. Your input, combined with my experience and knowledge about you, helps me craft the best letter I can to support your application!
